IRADAH’s Outreach and Home-Based Support program takes rehabilitation services directly to the doorsteps of persons with disabilities, ensuring that even the most remote and marginalized individuals are not left behind. Many families in rural Chakwal face significant barriers—such as distance, lack of transportation, and financial constraints—that make it impossible for them to access specialized services. This program overcomes these obstacles by bringing expert care and guidance into their homes.

The outreach team consists of a qualified female physiotherapist and a trained counsellor who regularly travel to surrounding villages. Their visits begin with a thorough check-up for the person with a disability, focusing on both their physical condition and overall well-being. Alongside the medical assessment, the counsellor engages with the individual and their family, offering guidance on coping strategies, emotional support, and ways to encourage independence.

A critical component of each visit is a detailed assessment of the home environment. The team examines how living spaces, furniture, and facilities can be adapted or improved to support greater mobility and self-reliance. This might involve simple yet impactful changes, such as adjusting the style of the toilet, providing a more accessible bed, or supplying a wheelchair, crutches, or other assistive devices. These improvements are recommended based on the specific needs of the person, ensuring that changes are both practical and sustainable.

When physiotherapy exercises are needed, the physiotherapist teaches them directly to the individual and, where possible, trains family members to assist with daily practice. This hands-on approach ensures that rehabilitation continues long after the team leaves, maximizing the potential for improvement.
